Registration
Thank you for your interest in the classes offered by Shirley Ford. In order to register please follow these simple steps.
1. Download the form and save it to your "My Documents" folder.
2. Fill out the form on the computer using MS word or another word processing program.
3. Return the completed form as an attachment emailed to godzcreation05@yahoo.com
4. Once you have completed these three easy steps, watch for additional information within the next three weeks.
